Examine and document the optic disc, macula, vessels and periphery by ophthalmoscopy or slit lamp, the way it is done every day. Attach a photo if you happen to own a camera.
Keratometry without a topographer
Capture flat and steep K and the axis from a manual keratometer, in the same chart as the refraction. Topography adds maps most routine visits never need.
